RESOLUTION AUTHORIZING THE CITY MANAGER TO EXECUTE THE CONTRACT
TO PROVIDE SCHOOL HEALTH NURSING SERVICE TO THE CITY VIEW INDE-
PENDENT SCHOOL DISTRICT FOR THE SCHOOL YEAR 1984-'85 AND PRO-
VIDING THAT THE CONTRACT BE RENEGOTIATED EACH YEAR.
WHEREAS, the City View Independent School District desires the public health
nurses of the Wichita Falls-Wichita County Health District to provide limited
school health nursing services to their schools; and
WHEREAS, the health district is capable of providing those services at a cost
of $15.75 per hour plus $0.20 per mile travel (roundtrip 10 miles); and
WHEREAS, the cost of providing this service and the amount of service required
each year will vary, a new contract must be negotiated each year.
NOW, THEREFORE BE IT RESOLVED BY THE CITY COUNCIL OF WICHITA FALLS, TEXAS,
THAT:
The City Manager is authorized to execute a contract with the City View
Independent School District to provide school health nursing services to the
City View schools at a cost of $15.75 per hour plus $0.20 per mile transportation
10 miles roundtrip) to and from the health department.
Change in cost and amount of services needed requires that a new contract be
negotiated each year.
This resolution shall become effective November 6, 1984.

CONTRACT BETWEEN THE CITY VIEW INDEPENDENT SCHOOL DISTRICT
AND THE CITY OF WICHITA FALLS TO PROVIDE SCHOOL HEALTH NURSING
SERVICE FOR REMAINDER OF THIS SCHOOL YEAR OF 1984-85.
Effective November 6 , 1984 the nursing service of the Wichita Falls-
Wichita County Public Health District will provide two (2) hours per week
of public health nursing service to the schools (one elementary and one junior
l;igh) in the City View Independent School District. The two hours will include
travel time to and from the health department.
It is agreed the school district will pay $15.75 per hour, plus $0.20
twenty cents) per mile (10 miles roundtrip) or $33.50 per week for the re-
maining 28 weeks of the school year, or $938.00.
It is also agreed that should additional time be required the service
will be provided for $15.75 per hour plus additional travel when indicated.
Additional time will be provided only upon approval of the school principal
or the Superintendent of schools.
It is further agreed-that the health department shall-render a statement
for services at the end of February and the end of May. The statement shall be
payable upon receipt and not longer than 30 days.
This contract shall be binding upon both parties unless altered or dis-
continued by mutual agreement and after at least 30 days from receipt of written
request for any change.
The effective dates of this contract extend from November 6, 1984 through
May 31, 1985.
