Res 132-87 8/4/1987RESOLUTION NO. 8,,z-d 7 RESOLUTION APPROVING FIRST AMENDMENT TO CONTRACT FOR AMBULANCE SERVICE TO WICHITA COUNTY AND THE CITIES OF WICHITA FALLS, BURKBURNETT AND IOWA PARK, TEXAS. WHEREAS, the Cities of Burkburnett, Iowa Park and Wichita Falls and Wichita County have entered into a contract dated July 24, 1987, with Life Line Emergency Medical Services, Inc. for ambulance service to such entities; and, WHEREAS, the parties desire to amend such contract with reference to the insurance required to be provided by Life Line Emergency Medical Services, Inc. NOW, THEREFORE, BE IT RESOLVED BY THE CITY COUNCIL OF THE CITY OF WICHITA FALLS, TEXAS, THAT: That certain First Amendment to Contract for Ambulance Service to Wichita County and the Cities of Wichita Falls, Burkburnett and Iowa Park, Texas, a copy of which is attached hereto, is hereby approved, and the City Manager is authorized to execute the same for the City of Wichit- Falls. i PASSED AND APPROVED this t - mg Li day of A •u t, 19:7. City Clerk FIRST AMENDMENT TO CONTRACT FOR AMBULANCE SERVICE TO WICHITA COUNTY AND THE CITIES OF WICHITA FALLS, BURKBURNETT, AND IOWA PARK, TEXAS This First Amendment made and entered into this the day of August, 1987, by and between the City of Burkburnett, Texas, the City of Iowa Park, Texas, the City of Wichita Falls, Texas, and Wichita County, Texas, and Life Line Emergency Medical Services, Inc. , a Texas corporation, WITNESSETH: WHEREAS, the parties hereto have entered into a contract dated July 24, 1987, providing for ambulance service; and, WHEREAS, the parties desire to amend such contract with regard to the insurance required to be provided by Life Line Emergency Medical Services, Inc. NOW, THEREFORE, the parties hereto do hereby agree as follows: Paragraphs B. , C. , and D. of VIII. Insurance of such contract dated July 24, 1987, are hereby amended, and Paragraphs E and F are added to Paragraph VIII. , to read as follows: B. The Contractor shall provide, at its own expense, the following insurance coverage: 1. Comprehensive automobile insurance coverage extended for fire, theft or any other physical loss of ambulance except by collision or upset. 2. Collision and upset insurance for all ambulances with value no less than current value of vehicle and contents with not more than One Thousand ($1,000. ) Dollars deductible. 3. Automobile liability insurance coverage as required by State Law as may be amended from time to time. Current requirements are in amounts of at least Five Hundred Thousand ($500,000. ) Dollars bodily injury per person, Five Hundred Thousand ( $500,000. ) Dollars bodily injury per incident, and Five Hundred Thousand ( $500,000. ) Dollars property damage. 4. An umbrella automobile liability policy in a minimum amount of One Million ($1,000,000. ) Dollars. 5. Professional malpractice liability policy in a minimum amount of Five Hundred Thousand ($500,000. ) Dollars. C. Insurance must be written by companies licensed to do business in the State of Texas. D. Contractor agrees to name the Entities as additional insured in the Automobile and Professional insurance policies. E. All insurance policies must contain a provision that the policy shall not ben canceled, modified, expired or otherwise terminated until after at least thirty (30) days written notice to that effect is given to the Entities. F. All insurance policies shall be in form and content satisfactory to the Entities and should be submitted to the City of Wichita Falls at the time of contract execution. " This First Amendment executed as of the day and year first set out above.
