I. CALL TO ORDER & INTRODUCTIONS
Chair Dr. Sutton called the Board of Health meeting to order at 12:15 pm after a quorum of members was
attained.
II. APPROVAL OF MARCH MEETING MINUTES AND ABSENCES
Dr. Sutton called for the review and approval of minutes from the last meeting held on May 20, 2011.
Bryan Press introduced a motion to approve the minutes as presented and Dr. Carlston seconded the
motion. Motion passed unanimously.
At this time it was noted the excused absence of Dr. Jansen, Dr. Hill, and Robin Moreno.
III. AMBULANCE ORDINANCE
Susan Morris, Environmental Health Administrator, and Kinley Hegglund, Senior Assistant City Attorney,
submitted the final revised Ambulance Ordinance for review after meeting with AMR, Trans Star, City
Departments, and stakeholders. The ordinance is to ensure excellent ambulance service that protects
the health and safety of the citizens.
Ms. Morris proceeded to present the changes.
42 -3. Qualifications for Franchise Holder
(2) Maintain at least 4 ambulances currently licensed by the State of Texas and based at its primary
facility
Trans Star maintains 4 ambulances requested the change from the previous number of 5 so complied
since no apparent reason was found.
42 -21. Insurance — Ambulances
(a) Before an ambulance permit is issued to operate an ambulance service in the city, the franchise
holder shall procure and furnish to the Director and shall thereafter keep in full force and effect the
following insurance coverage as reguired by the State of Texas.
Cleaned up the language deleted the dollar amounts inserted as required by the State of Texas.
(b) Every insurance policy shall insure all of the ambulances owned, leased, contracted for or controlled
by the holder of such franchise and used in such ambulance service for which a franchise has been
authorized. Such policy shall inure to the benefit of any person who shall be injured or who shall sustain
damage to property caused by the operation of such vehicles.
Deletions:
(1) At minimum, automobile liability insurance coverage that complies with State laws.
(2) Professional malpractice liability insurance coverage in a minimum amount of $500,000.
The malpractice liability insurance is addressed in the current contract.
(d) The franchise holder shall keep current insurance certificates covering all of its vehicles on file with
the Director during the term of the franchise and shall cause current evidence of insurance for each of its
vehicles to be kept in said vehicle. Insurance certificates and policies shall name the City of Wichita
Falls as an additional insured, provide the City with at least 30 days notice of cancellation, non - payment
of premium, or non - renewal, and include a waiver of subrogation in favor of the City.
Deletion of the glove box of each since an ambulance does not contain a glove box.
42 -31 Ambulance permit
(a) The franchise holder shall obtain an ambulance permit for each ambulance utilized in his ambulance
service, indicating the ambulance's authority to operate, which shall show thereon the date of expiration
of the permit for the ambulance. An ambulance permit must be attached to the left rear window of the
ambulance in a manner approved by the Director. The franchise holder must pay a permit fee for each
ambulance permit he obtains from Director.
Windshield being in the front was replaced with window.
Fees for Franchise Applications, Franchises, and Annual Permits moved from the ordinance into the
Health Department Fee Ordinance.
Initial franchise application fee is 500
Annual franchise fee is 100
Annual permit fee per ambulance is 100
Ms. Franklin included that Ms. Morris called other entities with ambulance services to inquire about their

fees. The fees are quite higher but had not been adjusted since the 80's. AMR and Trans Star wanted
higher fees, but after discussion with Mr. Hegglund there was no justification.
Dr. Sutton introduced a motion to recommend the ordinance based upon the change of its to a in section
42 -3. Qualifications for Franchise Holder
To apply for a franchise, an applicant must:
(1) Have its primary facility located in the territorial limits of Wichita Falls;
The motion to recommend was made by Dr. Carlston and seconded by Dr. Clark.
Ms. Franklin intends to take the ordinance to Council on August 2, 2011.

V. ADJOURN
Dr. Sutton requested a motion to adjourn. The motion was made by Dr. Clark and seconded by Dr.
Carlston. The motion carried and the meeting was adjourned at 12:45 p.m.
