MEMORIAL PARK has subdivided the real property described as:
That certain called 178 acre tract of land as described in the instrument recorded in
Volume 457, Page 363 of the Deed Records of Wichita County, Texas. Said tract being
situated in the Green Reynolds Survey, Abstract 251, of Wichita County, Texas and hereby
designates said plat as burial property consisting of CRESTVIEW MEMORIAL PARK, THACKER
FAMILY ESTATE and hereby declares that said plat sets forth the location and gives the
dimensions of the lots, spaces, niches and utility easements consisting the some, and
that each lot shall be known and described by number assigned to each one respectively
on said plat.
SCI TEXAS FUNERAL SERVICES, INC., A TEXAS CORPORATION, dbo CRESTVIEW MEMORIAL
PARK dedicates the real property to be used exclusively for cemetery purposes, including any
and all purposes necessary or incidental to establishing, managing, operating, maintaining,
improving, or conducting a cemetery, interring human remains, including cremated human
remains, or caring for, preserving and embellishing cemetery property.

executed by its President or Vice —President and its Secretary or another person authorized
by the Board and acknowledged.
This dedication is made pursuant to Section 711.034 of the Texas Health and Safety
Code and its constructive notice of the terms and provisions hereof.
